Preeclampsia Foundation Research Questionnaire
The Preeclampsia Foundation is committed to the
dissemination of information about preeclampsia with the goal of preventing
tragic outcomes of pregnancies and saving the lives of mothers and their
babies.
This survey has been designed with input from medical experts in the field to
help collect information that can be used to inform health care providers,
researchers and expectant families about preeclampsia.
Your answers to the survey questions may eventually help prevent other families
from experiencing the grief and loss that may result from this condition. We
appreciate your taking the time to assist in this important effort.
The
requirements of this survey are that you, or the person you are filling
out the survey for, has had an INDEX pregnancy.
The INDEX pregnancy is the first one where there was an adverse outcome or
there seemed to be a risk for an adverse outcome. There may have been a prior
pregnancy with preeclampsia where it was not diagnosed or there were no
resulting problems. The INDEX pregnancy to which the questions refer should be
the one where there was awareness that preeclampsia could become a serious
problem.

individuals who visit the Preeclampsia Foundation’s website.
This information may expedite the development of more effective treatments
of preeclampsia and other hypertensive disorders of pregnancy .

to the Preeclampsia Foundation, and we are committed to protecting such
information against unauthorized use or disclosure.

strictly confidential and will not be disclosed, given, sold or transferred to
any third party, including medical researchers, without your written consent.

participating in this survey you are authorizing the Preeclampsia Foundation to
share non personally identifying information collected as part of the survey
with researchers who are studying preclampsia and/or other hypertensive
disorders of pregnancy.

in the survey is completely voluntary and is of no cost to you.

will never be contacted by any party, other than a representative of the
Preeclampsia Foundation.

participation in this survey does not imply membership in the Preeclampsia
Foundation.
