Crystal, here's what they mean by "index pregnancy" when you are taking the survey:

The INDEX pregnancy is the first one where there was an adverse outcome or there seemed to be a risk for an adverse outcome. There may have been a prior pregnancy with preeclampsia where it was not diagnosed or there were no resulting problems. The INDEX pregnancy to which the questions refer should be the one where there was awareness that preeclampsia could become a serious problem.

What is an INDEX pregnancy. I tried to take the survey but I don't really know what you mean by INDEX.
