Use the Save button to save the document on your computer, fill it in, print it and then fax or mail it.

Costs include: $140 for everything

$30 for dinner opening night (per person)
$50 for the conference all day
$15 for lunch
$15 for picnic dinner
$30 for a sweatshirt (please specify size) S M L XL 1X 2X 3X

You cover your transportation, accomodations and breakfast.
