Okay guys, I'm now in the category of having had severe preeclampsia (albeit close to term) and an early miscarriage. I have been tested for the following:
PT (clotting time) was 10, INR .7 - both normal
Factor VIII was normal
TSH was normal
ANA was negative
Factor V was negative
Homocysteine was normal
Protein S was normal
Antithrombin III was normal
Protein C was normal
D-dimer was normal
I was also told that I am PAI 4G/5G.

The PAI 4G/5G thing is really just a polymorphism that is evident in about 50% of the population. Since 50% of the population don't get PE or miscarry, I and the hematologist I have seen feel that it really isn't the root of the problem. It's possibly a cofactor to something else.

Do you guys see anything else I should ask to be tested for? Could both of these things just be flukes? 10% of first time pregnancies develop PE and around 1/6 people miscarry. Was I just lucky enough to have both and they're completely separate events? I'm inclined to believe they are related, but I just don't know.

I'm really just not sure what to do at this point -- I've had 2 out of 2 pregnancies with problems now. Maybe 3rd time is the charm? Maybe we should just stop now and be happy with the one awesome child we've got?

I meet with my hematologist tomorrow to discuss what tests I've had run and any others she might recommend. I wondered if you guys would suggest anything you see that is missing.

I was so excited to be embarking on the journey to the next child and hopeful I would dodge PE. Instead I suffered a very early loss (which I know can be just one of those things), but I'm very discouraged now.

Any advice would be appreciated.
Thanks
Amy

Hello Amy, I think We are sailing in the same boat..Almost all the tests done on me till today are negative and Doctors seem to have no answer rightnow and I am not taking 'no' for an answer. I see many ladies here who have almost same tragic experiences of infertility, Miscarriages and severe PE.I am sure that these things are related to each other but Doctors dont know the cause yet...I hope we get some hopeful answers soon.
I really know how you are feeling rightnow as I am feeling the same :(.I am sorry I dont have any advice for you but just wanted to say ..That you are not alone and dont lose hope..

Amy, it might be worthwhile to be tested for antiphospholipid antibodies (anticardiolipin, lupus anticoagulant, and Beta 2-glycoprotein I). They can be a cause of PE & miscarriage.

Jenn, Self-Proclaimed Antiphospholipid Syndrome (APS) Guru
Miscarriages at 14 weeks & 6 weeks
Angel Maggie stillborn 10/15/00 due to HELLP Syndrome at 19.5 wks
Gabriel Easton, 9/5/05 at 31.5 wks--PPROM
Gideon Orion, 5/1/08 at 29 weeks--mild PE and PPROM (Darn those membranes!)
Stick a fork in me. I'm done.

I saw the hematologist today and discussed the early miscarriage and my history with PE. She thinks that the miscarriage had nothing to do with a clotting problem (I think based on it being so early), and it was just "bad luck". She is still recommending the LDA for pregnancy.

Then I went over all of my tests with her and she also noticed that the APS test was missing. She called my OB b/c she thought that it had already been done. My OB thought it had been done too b/c it is part of the panel that is always done for coagulation issues. (I had the coagulation panel done by my regular doc).

So, she's doing the APS test and we'll see what that says. She's pretty certain with my particular history that I don't have APS, but she was all for checking it for peace of mind. Also, if I do have it that would change her recommendation for my pregnancy.

So, now what's going on is this: next week I get my blood test to be sure the miscarriage was complete and I'll also get the APS test back. If it's fine, we just wait another cycle and try again. If it's not fine, we wait 8-12 weeks and retest (apparently the levels can go up and down). If it's fine the 2nd time, we do nothing different. If it's not fine, then we would be discussing Lovenox for the next pregnancy. At that point, we'll be doing some soul searching to decide if we want to go down that road or not.

So, we're in waiting mode -- I'm just ready to get on with this already! I'm ready for another baby!!!

One thing that is possible my OB and Hematologist both said is that there may be undiscovered thrombophilias, hemophilias, antoimmune disorders or genetic variances that can contribute. It was not too long ago when no one knew about PAI 4G4G, etc, MTHFR, and the like. So who knows, maybe it is yet to be discovered. The most promising thing is that there is now so much more research being done than before; it has to be just a matter of time before some definative causes surface (ok, so I am being very hopeful, but you have to admit it is possible).
