Early pre-e or something else?

Hi everyone,

I am 22 wks w/ #3 & had pre-e with my first 2 pregnancies. Thankfully, I ultimately had good outcomes with both my children as they are both healthy and thriving now. I know each pregnancy is different but #3 has been totally different from my others. I spent my first trimester with almost daily migraines no matter what I did. The migraines have improved (1x/wk compared to 5x/wk) but I still "don't feel right". My non-pregnant bp is usually around 115/70ish. Now it's more in the 130/85 range (sometimes higher, sometimes a little lower). I don't know about my protein because my dr doesn't usually tell me (I think to keep me from worring). But what's really different this time is my heartrate. Non-pregnant it usually hovered in the 75-85 range resting. Lately it's been in the 100's with periods of it racing up into the 150's for 30min to an hour. I've never had this before. I know elevated heart rate can be normal but this seemed more than elevated. I finally called my dr and it took them days to get back to me. He finally ordered some blood work to check cbc, thyroid, & magnesium levels. I did the test this morning & won't find out anything until next week & I may still be referred to a cardiologist. So that leaves several days to wonder :? ... Is this some weird early pre-e manifestation or something else entirely? Could it be because I started this pregnancy about 40lbs heavier than previous pregnancies? Anyone else experience anything like this?

Oh I forgot to mention I've also had sharp pinching pain in my right shoulder. Sometimes it feels like it's stabbing from front to back & other times feels like someone is pinching my collar bone on the right side REALLY hard. It doesn't feel like muscle pain & nothing I do seems to help it go away. Deep breaths make the pain sharper. Today I've had that pain all day :(

Because you mention referred shoulder pain I would want to run bloodwork labs, *today*, to rule out the possibility that your liver is inflamed. It's very unusual to develop HELLP in a third pregnancy, but stranger things have happened. Can you go to L&D, report shoulder pain and a history of preeclampsia, mention the possibility that it's referred from your liver, and ask them to run a panel?

It sounds like you've developed tachycardia - pulse elevated above 100 - which is often treated with beta blockers. That alone would make anyone miserable, but on top of that you've got awful headaches and elevated pressures. Given the symptoms and your history, I would also be worried. Are you being seen by a maternal-fetal medicine specialist who has experience with these pregnancies?

I agree with Caryn. Please get checked asap and get some labs drawn. What caught my attention is that you described the right side pain as a stabbing pain from front to back.
Thinking of you and let us know how things are going.

I did get labs done this morning which included a cbc. Wouldn't that show if HELLP was developing? Or is there some other test I should ask for? I know that if the tests come back with bad results, the doctor's office will call. I don't know much about HELLP but I'll look it up. The only thing I knew about hellp is that you get epigastric pain, which I don't have. All I've had is gas today, lol. Not sure if gas can cause this kind of shoulder pain. (Compared to hellp, I hope it does!) I am not currently seeing a mfm. I thought about going to 1 but I really do like & trust my ob. This does have me second guessing whether I should see an mfm. I'll definitely talk to my ob about it. Ironically, after I got home from work today I took my bp & it was picture perfect 121/78, pulse 91. Go figure!
Thank you ladies so much for the input. You help me realize I'm not crazy or over reacting. No one has a greater stake in my health than I do.

Thanks Julija. I read that post & feel that is not what I am experiencing. I don't know what's going on with my crazy pregnant body right now but I'm glad I got the bloodwork yesterday and will be seeing my dr soon. Hopefully whatever it is, it will be nothing serious. I hate pre-e. I hated being on bed rest & would love to enjoy this last pregnancy and to be normal.
